The original title refers to a malicious spirit from Japanese Mythology called the Kuchisake-onna
and a modern version of a woman who, after being mutilated by her abused son, returns as a malicious spirit bent on committing the same act to her victims. For this film, Kôji Shiraishi
introduced a new aspect to the legend by addressing current social issues of motherhood, single mothers and child welfare.
The victims would die as she disfigured their faces to match hers.
Thirty years later, children are disappearing again, one by one. The slit-mouthed woman seems to have returned, but why? Why now after a long absence?
Schoolteacher Kyoko Yamashita (Eriko Sato
) sets out to investigate, to ease her guilt for failing to protect one of her students from the mysterious killer. She eventually discovers the origins of the slit-mouthed woman with Noboru Matsuzaki (Haruhiko Kato) at her side.

Plot
Legend holds that 30 years ago, a suburban town was terrorized by the spirit of a woman whose face had been grotesquely disfigured. Roaming the streets wearing a long raincoat, mask, and carrying large scissors, the spirit would approach her young victims and, as she removes her mask, ask "Am I pretty?"The victims would die as she disfigured their faces to match hers.
