Plot

The show starts with J.D. making a video postcard for his mother, but gets locked in a water tower for the day by the Janitor. Most of the show is then narrated by the Janitor. During the episode, he helps Carla with a patient by holding down his legs while the patient is having a seizure. After being thanked, the Janitor starts to feel important around for once, and also doesn't feel like acting like a jerk. That's until he overheard a conversation between two residents about how the whole thing is going to his head. Throughout the episode, he was talking to a disabled man who can't talk or move, and can only communicate with a computer that broke early in the episode. While the disabled patient was waiting for a new computer, the Janitor starts to talk to him to comfort him. He then starts talking about his problems, and that he thought he was part of the Hospital staff for working here. He soon released J.D. after Dr. Cox came back (as J.D.'s break was over), and the narration went back to him.

The subplot is about Elliot

 argument over an intern's mistake. The other subplot is about Turk being embarrassed by his impending fatherhood when Dr. Cox accuses him of having a stereotypically white personality.

The episode ends with Dr. Cox plugging in the disabled man's new computer. The man responds with "Thank you", which Doctor Cox thought was for him. The man quickly says "I wasn't talking to you," and Cox looks at the Janitor. Janitor simply nods and walks off.
