律令制下では怡土郡（いとぐん）と志摩郡（しまぐん）に分かれていたが、1896年4月1日、郡制に基づき両郡を合併させて糸島郡となった。現在の郡名は合併前の両郡の名前をつなげて別の字を宛てたものである。旧郡名の由来についてははっきりしないが、怡土については、弥生時代にその存在が魏志倭人伝に記述される伊都国との類似性が地理上と音韻上の両面から指摘されており、志摩についても、島との音韻上の一致が指摘されている。さらにこれらを裏付けるように、旧怡土郡地区からは大量の副葬品を伴う平原遺跡などの弥生時代の遺跡が何ヶ所か発見されており、同じ弥生時代には旧志摩郡地区も、ごく一部を除いて旧怡土郡地区とは船越湾から今津湾にいたる水道によって分離された状態であったと考えられている。
