, Milligan grew up in St. Anne's as a kid playing football on the street. He started his footballing career as a young boy playing for his local team, Lytham St. Annes Y.M.C.A., with fellow future professional football players Gavin McCann
and John Hills. When he was playing for Y.M.C.A., Everton Academy signed him, Gavin McCann and John Hills.

. His stay lasted only one month and on 5 September he moved to Leigh RMI. Before moving to Fleetwood Town in 2006, Milligan played for Hyde United where in August 2004 he was sent out on loan to Fleetwood after recovering from cruciate knee ligament damage.
Toward the end of the 2004-05 season
Milligan played for Blackpool's reserve team, making eight appearances, hoping to earn a new deal at the club. Still, in May 2005 then Blackpool manager, Colin Hendry
confirmed that he would not be offered a contract.
Milligan is Fleetwood's club captain. He was leading scorer in the 2007–08 season
with 21 goals as the club won promotion, as champions of the Northern Premier League Premier Division

. He won both the fans Player of the Season and the players' Player of the Season. On 23 May 2008 he signed a new three year contract at the club.
