Lucky eventually asks Elizabeth to be his girlfriend, and she accepts. One year after Elizabeths rape, on Valentines Day 1999, Lucky takes Elizabeth to a little church where they exchange vows. However, their relationship comes to a screeching halt on April 23, 1999 when Lucky is presumably killed when a fire is started at Jason’s bike shop leaving Elizabeth devastated. During Luckys funeral, it is revealed that Lucky is actually alive, and being held captive.

Helena later gives Lucky a diamond and instructs him to propose to Elizabeth. Nikolas realizes that Helena is controlling him again and tries to help his brother, but the two get into a fight in the stairwell at General Hospital which ends in Stefan Cassadine (Stephen Nichols
) falling down a flight of stairs. Stefan presses charges on Lucky and has him arrested. Luke and Laura send Dr. Collins to help Lucky again but this time he is unable to. After he escapes, Elizabeth finds Lucky and convinces him to get help. When Lucky almost kills his cousin, Lucas Jones
, he realizes something is wrong and he must get help.
After Lucky makes a pass at Nikolas’s girlfriend, Gia Campbell (Marisa Ramirez
), Nikolas kicks him out. Lucky notices a growing closeness between Nik and Elizabeth, he tells Elizabeth they are better off apart. Soon Elizabeth kidnaps Lucky to keep him safe from Helena and they confess their love for each other. Nikolas tricks Helena into showing him how to trigger Lucky’s programming. Meanwhile, Nik and Liz fake her death in order to make Helena believe he is loyal. Helena takes several Port Charles residents hostage in her secret lab and tries to get Lucky to kill them, but Lucky manages to break through the programming by focusing on his love for Elizabeth.
Free from Helena, Lucky and Liz reunite and Lucky hides that Helena forced him to forget his love for Elizabeth. Not wanting to hurt her, they plan a New Year’s Eve wedding. When Gia tells Liz the truth, the wedding is called off and the couple tried to start over. When Liz and Gia are injured in a car accident, Lucky stays by Elizabeth’s side, but he still wasn’t in love with her. Soon, Liz’s sister, Sarah returns and she & Lucky develop feelings for one another. Lucky is furious when he learns that Gia caused the car accident and Nikolas helped Helena escape prison to keep the secret. Liz later finds out and breaks up with him for hiding it from her. (then Robyn Richards) who is upset by Lucky’s rejection tells Elizabeth the truth about Sarah and Lucky’s secret relationship. Sarah soon realized that she couldn’t compete with Lucky’s love for Elizabeth and left town.

Elizabeth later learns that she is pregnant but keeps her infidelity a secret while Lucky goes to rehab; despite that, he the couple divorces in January 2007. A paternity test later confirms that Jason is the baby’s father, but to protect the baby from Jason’s mob lifestyle, she claims Lucky as the father. Elizabeth also hopes to keep Lucky on the straight and narrow by giving him something to look forward to, like a child. Liz and Lucky remarry on March 22 and Elizabeth welcomes her son, Jacob Martin Spencer in May 2007.
Elizabeth seeks Jason's help when Jake is kidnapped and their continued friendship causes both Lucky and Jason’s girlfriend, Sam McCall (Kelly Monaco
) to become jealous. Angry with Elizabeth, Sam then worked hard to get Lucky's attention as payback, and they began a steamy affair. In November 2007, during the Black and White Ball, Lucky discovered the truth about Jake's paternity and agreed to keep the paternity a secret to protect Jake; but the couple divorced shortly after. Meanwhile, they mourn the death of their close friend, Emily Quartermaine (Natalia Livingston

On Valentine’s Day, Liz returns to the church where they first married and passes out from hypothermia. Lucky finds her and takes her to the hospital where they learn she is pregnant and either Nik or Lucky could be the father. An overwhelmed Elizabeth attempts suicide but she is stopped and Lucky, Nikolas & her brother, Steve

) agree that Elizabeth needs helps and she checks herself into Shadybrooke. On March 17 it is confirmed through a DNA test that Lucky is the child’s father, but Helena Cassadine returns and alters the test results to make Nikolas appear to be the child’s father. On July 19, 2010, Elizabeth gives birth to another boy, Aiden Alexi Nikolossovich Cassadine.

) take Jake into surgery while everyone else awaits news on his condition. However, his injures are so severe that Jake flatlines, and is left on life support. Jason breaks the news about Jake to Lucky. He, in turn, tells Liz, and both are devastated. Elizabeth is furious with Jason when he asks her to donate Jake’s kidneys to Carly Jacks’s (Laura Wright
) daughter Josslyn who is suffering from advanced kidney cancer. Lucky eventually talks Elizabeth into letting Jake go in order to save Josslyn and other children like her.
On March 28, it is learned that Luke Spencer was the driver who hit Jake. Luke begins on a downward spiral, and the Spencers (including Liz) embark on an intervention which doesn’t go too well. In April 2011, Maxie Jones offers her condolences to Elizabeth and accidentally discovers the DNA test and tries to convince her to tell Lucky and Nik the truth. After hearing about Lucky’s upcoming wedding to Siobhan McKenna

), she decides to tell him the truth but stops herself. Liz soon finds the present Lucky was planning to give Jake on his birthday and returns it to Lucky in tears, interrupting his wedding night. After the fall from Luke skipping out on rehab and rejecting his family, Lucky almost returns to drinking and Elizabeth forces herself to tell him the truth hoping to give him something to live for. Lucky is overjoyed but Nikolas doesn’t accept the truth as easily and he’s leaves town.
In July 2011, after Liz goes back to work, she assist in Siobhan’ surgery and accidentally gives her the wrong drugs, almost killing her. Siobhan believes Liz tried to kill her and sues her for malpractice until Lucky convinces her to drop the lawsuit. Meanwhile, Lucky is assigned to a case when prescription drugs are going missing from the hospital. Though Siobhan doesn’t think Lucky can handle being around prescription drugs, Elizabeth encourages him. When Lucky’s cover is blown, mob boss Anthony Zacchara (Bruce Weitz
) gets his dealer inject Lucky with some drugs and leaves him in an alley. After a series of hallucinations, Liz finds him and tries to talk him down. Lucky tells Elizabeth that she is the only one he will ever trust. When she realizes how high Lucky is she leaves him in the care of Lulu’s boyfriend, Dante Falconeri

) so she can go to the hospital and get medicine to help Lucky. However, a hallucinating Lucky knocks Dante out and runs off to the church where he and Liz took their vows in 1999 and begins hallucinating again. Elizabeth appears in the church, as a hallucination, while Lucky simultaneously flashes back to their 1999 vows. Lucky begins to talk to his hallucination, telling Elizabeth that what they have together is a "permanent lock" (referencing their 1999 vows), but he cannot figure out how to stop things from collapsing every time they reunite. When the hallucination disappears, Lucky passes out in the church. Meanwhile, Elizabeth gets into a confrontation with Lucky's wife, Siobhan, and Siobhan falls down a flight of stairs. While driving Siobhan to the hospital, Liz loses control of the car while try to keep Siobhan conscious and collides with Jason and Carly, who are rushing to Carly's daughter, Josslyn. A accident occurs and Elizabeth is the only person uninjured from the accident. She goes to the hospital and begins to talk with Dante about the accident, when she suddenly realizes where Lucky went. She races to the church where they took their vows when they were younger, and injects Lucky with medicine to counteract the drugs he was given. Lucky wakes up with Elizabeth by his side, and she later tells him about Siobhan and takes him to the hospital. Siobhan survives the surgery, but later dies after she is poisoned by Anthony Zacchara (Bruce Weitz
). Not being able to handle any more loss, Lucky relapses and starts taking pills again, but Elizabeth continues to have faith in him.
In mid-October 2011, Elizabeth wents to get back with Lucky, however, he feels that every time they get back together it turns "toxic".

Early Years, Lucky’s Death & Helena’s Control
Lucky and Elizabeth meet for the first time on August 11th, 1997. Though she hides it well, Liz has a huge crush on Lucky from the start. Lucky, however, does not return the feelings. Instead, he is more interested in Elizabeth's older sister, Sarah Webber. Lucky begins to casually date Sarah, in the meanwhile forming a playful friendship with Elizabeth. Elizabeth continues to crush on Lucky, but he remains oblivious. Everything changes in February 1998, when Lucky invites Liz to a Valentine’s Day dance. Though Lucky only meant to invite Liz to join him and his group of friends, Elizabeth believes Lucky has asked her on their first date. When Liz finds out that Lucky is attending the dance with Sarah, she decides to skip it. Lucky becomes suspicious of why Liz wasn't at the dance, and decides to go look for her. Soon Elizabeth kidnaps Lucky to keep him safe from Helena and they confess their love for each other. Nikolas tricks Helena into showing him how to trigger Lucky’s programming. Meanwhile, Nik and Liz fake her death in order to make Helena believe he is loyal. Helena takes several Port Charles residents hostage in her secret lab and tries to get Lucky to kill them, but Lucky manages to break through the programming by focusing on his love for Elizabeth.
Free from Helena, Lucky and Liz reunite and Lucky hides that Helena forced him to forget his love for Elizabeth. Not wanting to hurt her, they plan a New Year’s Eve wedding. When Gia tells Liz the truth, the wedding is called off and the couple tried to start over. When Liz and Gia are injured in a car accident, Lucky stays by Elizabeth’s side, but he still wasn’t in love with her. Soon, Liz’s sister, Sarah returns and she & Lucky develop feelings for one another. In February 2009, when Liz is poisoned after a toxic gas is released at General Hospital, Lucky stays by her side. The two decided to stay friends and co-parents, fearing that a romantic relationship wouldn’t work. When Lucky began dating Rebecca Shaw, Emily’s long lost identical twin, it bothered both Nikolas and Elizabeth. Nik and Liz kissed but realized a relationship would be too awkward because they’ve been friends for so long. Soon Lucky admits that he couldn’t sleep with Rebecca because he felt like he would be betraying her and the two begin dating once again. However, a hallucinating Lucky knocks Dante out and runs off to the church where he and Liz took their vows in 1999 and begins hallucinating again. Elizabeth appears in the church, as a hallucination, while Lucky simultaneously flashes back to their 1999 vows. Lucky begins to talk to his hallucination, telling Elizabeth that what they have together is a "permanent lock" (referencing their 1999 vows), but he cannot figure out how to stop things from collapsing every time they reunite. When the hallucination disappears, Lucky passes out in the church. Meanwhile, Elizabeth gets into a confrontation with Lucky's wife, Siobhan, and Siobhan falls down a flight of stairs. While driving Siobhan to the hospital, Liz loses control of the car while try to keep Siobhan conscious and collides with Jason and Carly, who are rushing to Carly's daughter, Josslyn. A accident occurs and Elizabeth is the only person uninjured from the accident. She goes to the hospital and begins to talk with Dante about the accident, when she suddenly realizes where Lucky went. She races to the church where they took their vows when they were younger, and injects Lucky with medicine to counteract the drugs he was given. Lucky wakes up with Elizabeth by his side, and she later tells him about Siobhan and takes him to the hospital. Siobhan survives the surgery, but later dies after she is poisoned by Anthony Zacchara (Bruce Weitz
