A shut toilet on the floor Elliot is usually on means that throughout the episode she uses the men's toilet. While she is in there, Dr. Kelso comes in talking on the phone to his wife about the fact that since Elliot threw him a birthday party - thus revealing to everyone that he was above retirement age - the Board had been trying to replace him. Elliot hears the door shut and so comes out to find that Dr. Kelso has tricked her and is still there.
Dr. Cox teases JD about his weakness, and says that he should be more of a role-model for his son. This leads JD to picking a fight with Turk, which he loses. Everyone in the staff room watches as JD is thrown onto the sofa and sat on by Turk. When JD tries to tell Turk to get off, he accidentally elbows him in the face, knocking him to the floor. He then pretends that he meant to do this and openly mocks Turk.
Turk chases JD across the hospital. JD then runs to the cafeteria where Carla and Elliot are sitting, because he knows that Turk wouldn't do anything in front of Carla. Carla and Elliot are talking about masculinity, when Carla
casually reveals that Turk lied about his "vacation", and had in fact had a testicle
removed.
JD fantasizes about growing another Turk from the testicle. When he is finished, he realises that Carla and Elliot have left, so runs away again. He then locks himself into a room. He then asks Turk about his operation, and Turk tells him it was after a bout of testicular torsion
, caused by a playful kick to the groin from Izzy. J.D. is hurt that Turk had kept this from him, eventually forcing Turk to admit that the surgery had left him insecure about his masculinity.
Meanwhile, The Janitor
starts a hospital newsletter, "The Janitorial", which airs the hospital's dirty laundry, including an article about Dr. Cox saying that he just needed a hug. Dr. Cox apologises for calling the Janitor stupid and begs him to remove the article.
Finally, Elliot
must a convince a male patient with breast cancer
to tell his macho brothers about the disease, which he refuses. Elliot and Carla retire to the men's bathroom to talk about the fact that men always say they don't want help when they need it most. Elliot tries to put this on her answering phone using her mobile, but Carla tells her you can't get any signal there. Elliot remembers Dr. Kelso's 'phone conversation' with Enid and realises that he was actually asking her to help him.

Plot
J.D.'s narration reveals that, since visiting his brother the month before, Turk has been acting more competitively than usual.
