The route has a signed eastern terminus at the intersection of Detroit Road and Wooster Road, which is also the signed terminus of State Route 113 and where a brief concurrency with U.S. Route 20

From there, it continues into Clifton before ending at the intersection of Clifton and West Clifton Road (the terminus of State Route 237 and the unsigned terminus of State Route 113), where U.S. Route 20 joins U.S. Route 6 and State Route 2 for a triple concurrency on Clifton throughout Lakewood.

The signed portion of State Route 254 on Detroit Road from the end of the State Route 2 concurrency to the eastern terminus is officially known as State Route 254 D.
