Levels

The game download includes only the original environment, Downtown, which is a sandbox level with 3 unlockable environments. Downtown also offers the modes Fun With Explosives, Spank The Monkey, Mime Toss, and Bowling. On November 24, 2010, Idol Minds provided long-time fans with a free environment called Hurt Falls, sponsored by AXE deodorant. It includes the mode Fun With Explosives.

The following DLC levels may be purchased:

Abusement Park, with one unlockable environment, and modes Fun With Explosives, Clown Toss, Hot N Cold, and Trauma.

Sore Spots, broken down into two environments, Morningwood High School and Area 69, each with one unlockable environment, and modes Fun With Explosives, PAINalympics, and Mad Science!

Touchmounds Movie Studio, with one unlockable environment, and modes Fun With Explosives, Cratetastic, and Bowling.

Stiffstonian Museum, with one unlockable environment, and modes Fun With Explosives, Spank the Monkey, and Bowling.

Alpine Ski Area, with the mode Fun With Explosives.

Two PAIN pinball machines are also available for download, which have elements of Abusement Park and Area 69.

Theme song

The Pain theme song was made by Chuck Carr, and is downloadable through the PlayStation Store.

PAINlabs

PAINLabs are small levels, one of which takes the place of the last one released every Thursday.
Some levels are just a place, like a village, with random people, while others are made of one
main thing. For example, fan tube that blows you higher, or sticky conveyor belt that drives you to explosives.

There is one thing common with all PAINlabs, is that separately from normal levels, like downtown or gym, the space in it is endless, and so is the camera movement. When you reach the end of the map, there's an infinite purple-orange layer of nothing.

gave the game a 6.8/10 commenting that "One level, two characters, no online multiplayer and no way to share crazy clips sucks. However, there are a ton of trophies to unlock."

Since the time of these reviews, several levels, online multiplayer, and new playable characters have been added.

As of October 26, 2010 a new patch was released for Move capabilities.
