Born Robert Otway, he was the only surviving son of Henry Otway and his wife, the 3rd Baroness Braye. His uncle was Sir Robert Otway, 1st Baronet, an admiral in the Royal Navy
. In 1818, he took the additional surname Cave by royal sign manual. Otway-Cave entered the British House of Commons
in 1826, sitting for Leicester

, which he represented until December. He was again returned for the constituency in 1835, a seat he held until his death in 1844.
On 23 October 1833, he married Sophia Burdett, eldest daughter of Sir Francis Burdett, 5th Baronet
. Otway-Cave died aged 48, after short illness without issue at Bath, Somerset.
