The woman, Eleanor Duncan, is married to Seth Duncan, the scion of the Duncan clan, which holds that part of Nebraska in its iron grip, keeping the population in docile silence. Reacher suspects domestic abuse, and coerces the doctor into doing the right thing.
Reacher's altruism sees him become embroiled in the Duncans' family business, and the unsolved disappearance of Margaret Coe, a child from twenty-five years earlier.
